Some sermorelin prescriptions arrive as a prepared liquid. Others may arrive as a powder with a separate diluent, the liquid used to dissolve it. Reconstitution means preparing that powder for use according to the dispensing pharmacy's instructions. It is not a step every patient needs to perform.
If the package does not make clear whether mixing is required, contact the pharmacist before opening or adding anything. The total amount printed on a vial, including 2 mg, 5 mg, 9 mg, 10 mg, or 15 mg, does not by itself specify a mixing volume or a dose.
A 5 mg or 10 mg vial label does not tell you how much liquid to add. Use the dispensing pharmacy’s instructions for that exact preparation.
Start with the actual prescription package
Check that the package includes your name, the dispensing pharmacy, the active ingredient or ingredients, and a readable instruction label. Compare the enclosed supplies with the instructions. A generic internet video may show a different concentration, diluent, or type of vial.
The FDA's compounding overview explains that compounded products do not receive the agency's premarket approval for safety, effectiveness, and quality. That makes product-specific instructions especially important. Historical directions for GEREF also cannot establish the storage life of a different compounded preparation.
Ask whether the medicine is ready to use, whether it requires refrigeration before mixing, and who will explain the preparation process. If your clinician and pharmacist provide conflicting directions, have them resolve the conflict before you proceed.
Why “how much bacteriostatic water” has no universal answer
The volume added determines the final concentration. A larger volume produces a more dilute solution for the same amount of drug. That changes how much liquid corresponds to a prescribed amount of active ingredient.
The correct diluent also depends on the product. Bacteriostatic water, sterile water, and saline are not interchangeable merely because they look clear. Preservatives and compatibility can matter. Do not substitute a household liquid, mix products together, or add extra liquid to stretch a supply.

Get a written preparation and storage plan
A useful pharmacy instruction sheet should answer these questions:
- Which supplied liquid is intended for this vial, and what exact volume is required?
- What concentration will the prepared medicine have?
- Which handling steps prevent contamination or damage?
- What date should be recorded when preparation or first use occurs?
- What storage conditions and beyond-use date apply afterward?
The answers should refer to the same prescription label and supplies in your hands. If the pharmacist changes the concentration, ask for new dose-volume instructions rather than using the old syringe mark from memory.
Appearance is only one part of checking a vial
Unexpected particles, discoloration, a damaged seal, or a leaking container warrant contacting the pharmacy before use. A normal-looking liquid does not prove that it is sterile or that its potency is unchanged. The FDA's risk explanation describes how contamination and incorrect strength can harm patients.
Do not try to rescue a questionable vial by adding more water, filtering it yourself, reheating it, or following a forum suggestion. Photograph the package and label for the dispensing team if they request that information, and keep it away from children while awaiting instructions.
Refrigeration and expiration need their own answers
A beyond-use date is tied to a preparation and its storage conditions. It is not interchangeable with a generic claim that all peptides last a certain number of weeks. The date printed on a diluent is also not necessarily the date for the mixed medicine.
